Why there is no form
A contact form would mean storing what you typed, so we do not have one. See the privacy policy for what does and does not happen to data on this site.
What we cannot help with
We cannot amend a certificate, arrange an assessment on your behalf, or answer for the register. A certificate can only be changed by the accredited assessor who lodged it, or by a new assessment. We can stop showing a certificate on this site — email the address above with the certificate number or the address.
